Full Job Description

AltaVita Assisted Living Centre is seeking a compassionate, enthusiastic nurse to join our team as the Wellness Director. This position requires an LPN license and at least five years of experience working with the elderly, preferably in an assisted living environment. The ideal candidate must exhibit genuine interest in our residents and patience and tact in dealing with residents, family members and visitors.

DUTIES WILL INCLUDE BUT ARE NOT LIMITED TO:

  • Assess health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident’s opportunity to remain in the assisted living environment.
  • Manages the health care of all residents, including the dissemination of information to families and staff.
  • Performs on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment and maintains contact with resident’s family with the view to returning to the community.
  • Completes a pre-admission assessment for each resident and updates every six months or as needed.
  • Initiates a resident service plan according to the individualized needs of the resident, as prescribed by physician and/or community policy. Provides resident and family instruction. Where applicable, seeks input from the resident and family in the development of the service plan.
  • Performs ongoing assessments of residents’ physical and psycho-social needs and coordinates with other departments to assure quality, proactive care.
  • Conducts pre-admission screening of prospective new residents. Assures that all required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms.
  • As appropriate, explains procedures and treatments to residents to gain cooperation, understanding and alleviate any apprehension.
  • Communicate with resident’s physician re: resident’s status and change of condition.
  • Provides training, supervision, and monitoring of staff in the administration of medications, to include frequently auditing medications sheets.
  • Hires, supervises, disciplines, and provides training and education to resident care staff on an ongoing basis.
  • Supervises the maintenance of residents’ charts, review documentation performed by the care giving staff.
  • Participates in training/orientation of new hires.
  • Responds to life saving situations based upon nursing standards, policies, procedures and protocol.
  • Participates in department quality improvement activities and quarterly quality management meetings.
  • Familiar with Colorado State Health Department rules and regulations pertaining to Assisted Living Residences.
